PLEASE NOTE THAT ADOPTIONS ARE WELCOME WITHIN 150-MILE RADIUS OF WOODSTOCK IL.

Thank you for your interest in adopting a senior pet! The following information is required so that we can assist you in the selection of your new pet. This form and consultation with a Young At Heart representative is designed to help us determine if the adoption is in the animal’s best interest, and to assist you in finding the animal most compatible with your lifestyle. Please be sure to read the pet's information completely before submitting an application. Please submit the completed application below.

THERE IS A $200* ADOPTION FEE FOR DOGS, and a $100* ADOPTION FEE FOR CATS. This helps us cover a fraction of the veterinary cost of getting a senior pet ready for a new home. A home visit may be required. A binding contract will be required to adopt the animal. Please note there may be other applications being considered for any given animal. Young at Heart pets are not placed on a "first come, first served" basis. Instead, homes are chosen based on the needs and the best fit for each senior pet.
